Fill out the form and get your immunization records (ask your Mom, ask your Dad, call 811 if you grew up in Alberta, call the health authority where you grew up.) (Really, call them, even if you call China.)

Scan and email your form and your records. SEND ALL IN ONE EMAIL.

No records? No problem, fill out the form and scan and email the form. (Give it an honest try to get those records.)

Check for an auto-reply. Look in your junk mail, spam, and all your mailboxes.

Wait for the nurse to email you and give you instructions.

Do what we say, go for blood work or get a chest x-ray.

Wait to get the email that says call us and book an appointment.

We are by appointment only, and we are very busy. We book a month in advance at times.
NOTE: If the nurse writes on your small white fold-out card or record "Due Sept 7 or later" you DO NOT have an appointment that day. She is telling you when to book. Call us and book an appointment on Sept 7, Sept 8, or Sept 9, etc. Remember we can not give you a vaccine a week, a day, or a minute early.
